Once submitted, this form will be forwarded to our product service department. A team member from that department will contact you with a RMA number and shipping instructions for your warranty/non-warranty return.

Please do not send any product until you have received authorization and a RMA from one of these Fleetwood team members. Items sent without a valid RMA may be returned to the shipper.
